Almost providing that I aim manually at the milkor .... the shot fails ......
plz help me or gimme a hint (im from colombia and my lv of english is 1)(It is not my fort ok!!!!)

I hear this somewhat, it's a tricky weapon when not used correctly.
First off, DON'T USE IT WHEN RIDING A WALL. If you fire parallel to a wall, there's a 1:3 chance with a grenade launcher that it will fail and hit the wall!
Also, watch the obstacles on the map. Grenades and grenade launchers can't shoot/be thrown over shrubbery in most cases!
